How can we improve our school?
This form serves as my 30 day notice to Go No Sen Karate of my cancellation of membership. I understand that my account will be cancelled 30 days from this notice and any charges incurred prior to the cancellation date are valid and fair. If charges are incurred for tuition during this time, I understand that it is my responsibility to use those classes within 30 days of the charge even if its past the cancellation date. Furthermore, I understand that I cannot seek a refund for any unused classes. However, any unused portion can be applied as an "In-house" credit towards camps and/or future events or when/if I restart membership.
