I, or the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of Spiders FC, its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with soccer and in consideration for the IYSA accepting the registrant for its soccer programs and activities (the ”Programs”), I hereby release, discharge and/or otherwise indemnify the IYSA, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs and/or being transported to or from the same, which transportation I hereby authorize. In addition, I, or the parent/guardian of the registrant, a minor, commit/agree on paying the full amount listed on this contract and Acknowledge that the registrant will be committing to Spiders FC per this training contract. In Addition No refunds will be made for cancellations due to inclement weather, injuries or other circumstances beyond our control. While all efforts are made to schedule and reschedule soccer events (practices, games, tournaments, etc.), Spiders FC cannot guarantee all events will occur due to variables beyond our control.

Parental Consent I hereby consent to let my child participate in the Spiders FC Soccer Training Programs and it is understood that every precaution will be taken for the safety and well-being of my child, but in the event of accident or sickness, Spiders FC, its staff and its volunteers are hereby released from any liability.
