BOTH PLAYERS MUST COMPLETE THIS FORM AND THEN A TEAM IS OFFICIALLY REGISTERED.
You may only play in Divisions that YOU AND YOUR GAMING PARTNER (if you have one) are both eligible for. Eligible age is determined by the age the player is on December 31, 2021. (Example: You are 10 years old, but turn 11 on December 30, 2021. You are officially ineligible to play in the 10 and under division, but can play in the Under 13, Under 16, and Under 18 divisions.)

Players may sign up for multiple tournaments, assuming both them and their gaming partner meet the age group requirements (Ex: Under 15). As a reminder, EACH PLAYER must register and pay for a team to become elligible. Thank you for your understanding.
