6:00-6:30PM We will be putting on glow bracelets, last minute costume pieces, taking photos, etc.

6:30-8:00PM Dance Party

8:00PM Pick Up

- Eat dinner beforehand
- Wear or bring layers in case your dancer gets too hot dancing in their costume.
- Pack water, masks, hair ties, etc.
- Bring clean sneakers or dance shoes in a dance bag for dancing.
- Feel free to wear your own glow wear.

Waiver of General Liability

I hereby release Downtown Dance LLC employees and independent contractors, from all liability from personal injury, illness or property damage occuring on or off the studio's premises. I authorize Downtown Dance to discharge my child to an ambulance with the nearest medical facility, should there be a medical emergency. I certify that my student is in good health and capable of participating in physcial activities. I hereby give permission to Downtown Dance to take and use photos and videos for the promotional uses of the studio. This includes, but is not limited to, the studio's facebook, instagram, twitter accounts and Youtube channel where students' names are not disclosed.
