Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by Jeunes
Diplomates during the selected camp. In exchange for the acceptance of said child’s candidacy by Jeunes
Diplomates ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve
and hold harmless Jeunes Diplomates . and all its respective officers, agents, and representatives from
any and all liability for injuries to said child arising out of traveling to, participating in, or returning from
selected camp sessions.
In case of injury to said child, I hereby waive all claims against Jeunes Diplomates . including all coaches
and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of
premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities,
including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis,
or death. /

Diplomates pendant le camp sélectionné. En échange de l’acceptation de la candidature de cet enfant par
les Jeunes Diplomates, j’assume tous les risques et aléas inhérents à la conduite des activités, et libère,
absous et libère les Jeunes Diplomates et tous ses officiers, agents et représentants respectifs de toute

Payment Procedure / Procédure de Paiement
The entire balance of the camp must be paid before the 30th of December of the year of the camp session
if I wish for my child to be registered. For registrations made after this date, the total amount must be paid
immediately. The number of places being limited, the first registrations received will be the first ones
enrolled. I will be notified as soon as possible. If my registration has not been accepted, my deposit will
be refunded in full. In case of emergency, I AUTHORIZE the person in charge to take all the necessary
actions in case of hospitalization (including surgical intervention). I agree to reimburse JEUNES
DIPLOMATES for the total amount of medical expenses. In the event of an accident, JEUNES
DIPLOMATES can not be held responsible. /
Le solde du camp doit être payé avant le 30 Avril de l’année de la session du camp si je souhaite que mon
enfant soit inscrit. Pour les inscriptions faites après cette date, le montant total doit être payé
immédiatement. Le nombre de places étant limité, les premiers arrivants seront les premiers inscrits. J'en
serai avisé le plus tôt possible.Si mon inscription n'a pas été acceptée, mon acompte sera remboursé en
totalité. J’AUTORISE le responsable à prendre en cas d’urgence, toutes les mesures nécessaires en cas
d’hospitalisation (y compris intervention chirurgicale). Je m’engage à rembourser JEUNES DIPLOMATES du
montant des frais médicaux.En cas d’accident, JEUNES DIPLOMATES ne pourra être tenu
