Effective October 15, 2021, Minneapolis Public Schools requires all volunteers in school buildings to comply with the following requirement:

Be fully vaccinated against COVID-19

or
agree to comply with COVID-19 weekly testing requirements
This is part of an MPS Board Resolution passed on September 14, 2021 requiring that all MPS staff as well as contractors, partners, and volunteers who have direct contact with students be vaccinated or be tested regularly in order to reduce the spread of COVID-19, as this is in the best interest and safety of our students, staff, families and community.
Definitions
Fully Vaccinated means at least 14 days after a second dose in a 2-dose series COVID-19 Vaccine or 14 days after a single-dose COVID-19 Vaccine.
COVID-19 Vaccine means a U.S. Food and Drug Administration or World Health Organization authorized or approved vaccine to prevent Coronavirus Disease 2019 (COVID-19). These vaccines include AstraZeneca/Oxford (Vaxzevria), Pfizer-BioNTech (Comirnaty), Moderna (Spikevax), Sinopharm/Beijing (Covilo), Janssen/Johnson & Johnson, Serum Institute of India/Covishield, and Sinovac/CoronaVac.
Weekly Testing Requirements means through community testing sites or other means, individuals who do not certify they are fully vaccinated, will certify that they’ve received a negative COVID test each week of direct student contact. The test can be saliva or nasal swab, antigen or PCR.
Direct Student Contact means being within 6 feet of a student (enrolled in PreK-12, Transition Plus or Early Childhood programs) for more than 15 minutes.
You are not required to supply this information, but if you decline to certify that you have been vaccinated or that you will undergo regular COVID-19 testing, that information will be shared with your supervisor and you may be prohibited from entering MPS buildings.
Your personally identifiable information is private, protected information, but may be shared with authorized MPS personnel who need the information to perform their jobs to ensure compliance with the Board Resolution, as well as for contact tracing. This may include staff in the Volunteer Department as well as the school principal.

Note: you will receive an emailed copy of your completed form after submission. Please keep a copy of the completed form (printed or electronic) and be prepared to show the form at the school check-in desk every time you enter the building. School staff will view the form but will not maintain a copy of your form at the school.
