Effective 10/15/2021, all Contracted Community Partners, as well as other Contractors with employees who have direct student contact (Organization), are required to certify that all Organization staff/representatives who enter an MPS building have been fully vaccinated. If an Organization's staff/representative declines to certify that they have been fully vaccinated, the Organization will ensure that the staff/representative complies with MPS requirements to test weekly for COVID-19.
This requirement applies to any organization that has a written agreement with MPS including (but not limited to) contractors, approved researchers, and lease holders.
Fully Vaccinated means at least 14 days after a second dose in a 2-dose series COVID-19 Vaccine or 14 days after a single-dose COVID-19 Vaccine.
COVID-19 Vaccine means a U.S. Food and Drug Administration or World Health Organization authorized or approved vaccine to prevent Coronavirus Disease 2019 (COVID-19). These vaccines include AstraZeneca/Oxford (Vaxzevria), Pfizer-BioNTech (Comirnaty), Moderna (Spikevax), Sinopharm/Beijing (Covilo), Janssen/Johnson & Johnson, Serum Institute of India/Covishield, and Sinovac/CoronaVac.
Weekly Testing Requirement means through community testing sites or other means, each Organization employee/representative assigned to MPS who does not certify they are fully vaccinated, will certify to the Organization that they’ve received a negative COVID test each week of student contact. The test can be saliva or nasal swab, antigen or PCR.
Direct student contact means being within 6 feet of a student (enrolled in PreK-12, Transition Plus or Early Childhood programs) for more than 15 minutes.
By signing this form, you certify that your Organization agrees to comply with the COVID-19 Vaccination and Testing requirement above. Failure to comply with this requirement may result in the termination of your contract with MPS.
Note: A copy of your completed form will be emailed to you upon submission. Please ensure that your employees/representatives have a copy of the completed form as they will be asked to show it when they check in at a school building.
