Remaining in Catalog Request

A new admissions application would still be required to capture current student information.
This form is to obtain approval to remain in an older catalog year when a student is required to submit a new application.

If requesting to remain in multiple degree programs, submit one form for each degree.
- Student should need no more than 12 credit hours to complete degree
- Courses no longer taught should be identified by the advisor to address potential substitutions
- New Application for Admissions Required. Prior to submitting Old Catalog Request Form
The Registrar's Office may seek departmental approval or feedback prior to providing a final decision.

The Registrar's Office and/or College Dean reserve the right to not allow students to remain in a past catalog due to rigor or requirements of the program.
