As the parent or legal guardian of the above athlete, I fully understand that participation in athletics, in this case track and field, could result in both minor and serious injury and even death.

I further understand that to minimize the risk of injury or harm, the athlete should have had a physical within the past year. I assume any and all risks associated with the above athlete’s participation as an athlete participating with Speed Matterz and Arkansas H.A.W.K.S. Track and Field. I hereby waive and release Speed Matterz and Arkansas H.A.W.K.S., its coaches and representatives of any and all rights for damages or injuries suffered while participating with Speed Matterz and Arkansas H.A.W.K.S. Track & Field during the 2021 season; while traveling to or from an event or during any other Speed Matterz and/or Arkansas H.A.W.K.S. related activity.

I also acknowledge that my child may be recorded for educational and promotional use and I do consent.
