The rights of Tribal NEW /SW AP participants:
- Participants will be informed that the case information is confidential and is shared only for purposes directly related to the administration of the program:
- To be informed in writing or orally regarding the status of your case, scope of the program, temporary focus of the program and related services available;
- To claim good cause if a sanction is imposed:
- To have his/her Civil Rights protected.
The responsibilities of Tribal NEW /SWAP participants:
- Providing information and verification as required.
- Complying with your Family Future Contract Activities.
- Providing timesheets by the 5th of each month.
My rights and responsibilities have been explained to me and I understand my participation requirements in the NEW /SWAP program. I understand that I am required to do my required hours each week and submit a timesheet reflecting these hours.
