  • All New Members Badges are automatically created using an BNI Atlanta's Application Log during the on-boarding process.  If the new member did not recieve a badge after 45 days after their induction to the Chapter, please use this form to help us correct the situation.

    IF a member needs a replacement badge or there were changes to their: 

    • Name Change
    • Comapny Name Change 
    • Classification Change
