As someone who is actively working to maintain a comfortable, trauma-informed experience and environment for each and every client, it is very important to me to give clients an opportunity to offer their feedback regarding their experiences in my space. I understand that some clients may find it difficult to voice their concerns, feelings, feedback etc during an appointment, and I want to ensure that I offer a space for every client to be heard so that I can continue to improve my practice and policies. This form is fully anonymous and will be used in my efforts to show up as the best version of myself within my practice.
