Complete this form if you have extreme circumstances and need to request a refund of all or part of the items purchased during registration for the 2023 National FFA Convention & Expo. Extreme circumstances include confirmed illness among attendees 10 days prior to or during the event and/or official cancellation of trip approval due to an illness. (Example: School administration cancellation of all student travel.) Proof and documentation of circumstances is required.
