  • Feeding Information

  • We recommend providing your pets regular diet to avoid any upset in digestion. For convenience, you may premeasure and individually bag the food, but not required. If the provided food supply runs out or you chose to use the boarding center Hills Science Diet food additional fees will be charged daily.

  • Medications

  • We are happy to give your pet his/her medications. Please list any medications your pet will need / is currently taking, include dosages and how often each medication is given. Please list the reason the medication has been prescribed.

    There is a medication administration fee charged per day your pet is medicated.

  • Belongings

  • We provide items such as toys, beds, blankets, food bowls, however, you are welcome to bring any of your pet's belongings from home at your own risk. We will make every effort to assure they are not misplaced but can not make guarantees.
  • Boarding Behavior

    Please complete the following and tell us a little bit about your pets personality and beahvior
  • All pets will receive a minimum of one-half hour of playtime per day. If they get along well with their housemates they can play together. If they do not, they will get individual play time with one of our boarding staff.

  • Companion Boarding Facility is pleased to offer bathing services for an additional charge to all of our boarders prior to departure. 

    Please realize bathing service availablity may be limited. Indicate below if your are interested in bathing services and our team will review service dates.

    Medications:  If your pet will be receiving medication during his or her stay, it must be in the original veterinary-labeled containers with instructions for administration. If over the counter medications are to be administered it must come in the originally labeled containers with instructions for administration.

    Vaccine Requirements: In order to protect the health of your pet, this facility requires documentation showing that all pets are current on vaccinations.  Pets that are so young that they have not completed their entire series of vaccinations may not yet be protected and, thus, owners accept any risks of infection.

    Vaccines are required as followed:                               

    Dogs:  Must be current on the DHPP/DHLPP and Rabies. They also must be current on respiratory vaccines including Bordetella, and Canine Influenza. If vaccines are not current, they will be administered by our veterinary staff upon arrival at the cost of the owner.

    Our facility is a flea free facility. In efforts to keep parasites away we require all pets to have a Negative Fecal within the last 12 months for those pets on monthly prevention, for pets not current on prevention they will need a negative fecal within the last 3 months. 

    Cats:  Must be current on the Rabies, FVRCCP, Feline Bordetella.

    If vaccines are not current, they will be administered by our veterinary staff upon arrival at the cost of the owner

    *Any animal in the facility found to have fleas will be administered an anti-flea medication at the owners expense.

    Statement of Kennel Policy   

    1. Mondays through Fridays Boarder intake & Drop off is between 9:00 a.m. and 4:00 p.m.  Saturday and Sunday pick up/drop off is by appointment only.

    2. This facility cannot guarantee the health of any animal, but pledges to provide appropriate care to all boarders.  I agree to hold this facility harmless for conditions that often are unavoidable in boarding environments, including, but not limited to, weight loss or gain, rough hair coat, kennel cough, upper respiratory infection, diarrhea, and fleas.     

    3. If my pet identified on this record should become ill, I request that Companion Animal Hospital provide all medical/surgical treatment it deems necessary, with fees not to exceed $_____________. I acknowledge that in the event of my pet’s illness, the staff may not be able to contact me immediately.  Nonetheless, they are authorized to initiate appropriate treatment until my agent or I can be reached. I agree to pay all related expenses associated with the treatment of my pet until I am available to discuss further care and related fees with the attending veterinarian.

    4. If pets are boarding in the same kennel we reserve the right to separate if problems arise.

  • Inclement Weather/Hurricane Release Form

  • The doctors and health care team at Companion Animal Hospital & Boarding Center never like to think that things could happen to your pets while under our care, but they can. Specifically in the event of a hurricane or other natural disaster, it may be beyond our control to keep your pet(s) healthy and safe from harm. Due to our location and the potential of inclement weather we have to be prepared. During such events it may be necessary or even mandatory that our health care team evacuate the premises and/or the area in the event of a Category 3 or higher storm. If an evacuation is necessary, all possible precautions will be taken to keep all pets in our possession safe. However, it is impossible to ensure that each pet/patient will survive a natural disaster of such magnitude. Please understand that we will do everything in our power to make sure all pets are taken care of to the best of our ability. Unfortunately, when a natural disaster endangers our health care team, we have to take precautions to ensure their safety which may leave us with no choice but to evacuate the hospital and boarding facility premises leaving animals unattended. We make no implied or expressed guarantee that our facility or designated evacuation site could withstand a disaster. 

  • By signing below, I,         , acknowledge that Companion Animal Hospital & Boarding Center doctors and health care team will not be held liable in the event of a natural disaster if any of my pet sustains any injuries or becomes deceased during that time. I realize that the doctors and health care team have to put their safety first, but they will try to ensure the safety of my pet to the best of their ability in the event of a natural disaster. I acknowledge that if I am unable to personally pick up or have and authorized person pick up/transport all pets listed below, he/she may be left unattended during the event of a natural disaster. If your pet is severely injured, and we are unable to provide advanced medical care for whatever reason, at their discretion, our veterinarians will humanely euthanasia your pet. Caring for your pet is our utmost concern; however we do not want any pet to suffer. If able to be evacuated, your pet will be temporarily housed in an air-line approved pet carrier or crate. The crate shall be solid on all sides with a grated metal door and holes for ventilation.

  • KENNEL COUGH/BORDETELLA & CANINE INFLUENZA

    URI Aknowlegment

  • Bordetella and Canine Influenza are two very contagious diseases that can be transmitted by inhalation of respiratory secretions. Dogs can spread these diseases at boarding facilities, dog parks, grooming establishments, veterinary hospitals, pet stores and any other animal related event. All circumstances listed can lead to the transmission and infection of these diseases. Dogs under stressful conditions are more susceptible due to suppressed immune systems.

    We recommend and highly encourage all incoming patients to the Boarding/Grooming Facility and any hospitalization to our hospital be fully vaccinated for these respiratory viruses. In addition we recommend both the Bordetella Vaccine and the Influenza vaccines be updated within 2 weeks of staying at either facility. Both Vaccines are required for admittance into our facility to decrease the chance of contagious diseases spreading in our hospital and boarding facility. 

    **If your pet receives either of the requires vaccines upon entry into our facilities we cannot guarantee that the vaccine has had ample time for immunity against these diseases to be stimulated. Neither vaccine is 100% protective. Similar to the human flu vaccine, which reduces the likeliness of contracting the flu and decreasing the severity of the symptoms if the flu is contracted. Both Bordetella and Canine Influenza can be spread by dogs that do not appear sick and are not exhibiting any clinical signs. There is no discerning factors in noticing which patients are and are not infected, for this reason it is most important all canines be vaccinated.**
    We are doing our part to ensure that bordetella and canine influenza does not contaminate our facility and we do require you do your part by properly vaccinating your pets. 
    However, we cannot guarantee that the carriers of the diseases are not present and therefore may transmit either disease or any other respiratory infection to your pet.
