Reviewing the options below and agree to each as appropriate. Completing this section will help you think through the structure of your program and will provide the Foundation with the details it needs to complete a brief YESS Group Agreement.
ENROLLMENT: Your check mark shows your agreement with each step in the cohort onboarding process.

Cohort Facilitator collects Application/Parental Consent Forms from all students and checks each form for Student Consents and general completeness.
If the student is under the age of 18, the Facilitator must obtain Parental Consents...or have them on file already for their curriculum for the current year. FACILITATORS DO NOT NEED TO SHARE CONSENT FORMS WITH THE FOUNDATION. The organization is responsible, however, for collecting and keeping them on file in case of complaints or legal action.
Facilitator leads the first class using the Intro Class PPT or related content. During the class, the Facilitator confirms all student names and emails and informs the students to expect an enrollment email within the next 48 hours. They should check Junk Mail and Spam if they do not see these emails in their In Boxes.
After the enrollment roster has been confirmed in the first class, the Facilitator submits the list to the Foundation following in an Excel CSV that includes First Name, Last Name, a unique email address, Birthdate (00/00/0000-month/date/year). Students will receive their enrollment emails within 2 business days after the list is received by the Foundation.

Graduation ceremony will be held virtually and will be organized by the Group admin, allowing the school to offer the program at the ideal date and time for the group and only for the group

Graduates will be asked to join the virtual ActionCOACH Foundation Quarterly graduation at the date and time set by the Foundation. Date and time may not be ideal for your group.
