    Evidences can be added in form of photos, audio files or videos, but the Max size limit is 5MB each. 

    The report gets sent back to the user on the user Email in this form.

    A comment is mandatory for every section, and every time you do not 'tick' a box.

    Dear Captain,

    In order to prepare with requisite documents at hand while the PSC inspection is going on please see the below list. Kindly make sure that the documents are complete, no missing pages, parts, and where required, updated to date.
    The list is not exhaustive but is general guidance so we are prepared as much as possible.

    Check and confirm that all trading and statutory certificates are original onboard and valid. Check all certificates for any missing endorsements – Annual /Periodical.

    It would be helpful if the following documentation could be assembled together in one location for inspection.

  • Note: In some ports, it’s a requirement if the gangway is not able to land on the Jetty then secondary support is required to be fitted to the gangway.

    • Accommodation / Housekeeping – Not in order. Toilet Flush not Working

    • Annual Ballast Pump Test Requirement not carried out.
    • Ballast Tank Air pipe vent Head / Float condition poor, stuck, missing or damaged.
    • Chemical Storage unsatisfactory, no proper segregation, Incomplete MSDS.
    • Structural Deficiency (Cracks, etc).
    • Helicopter Operations (Deck Strengthening Certificate from Class or Yard / FFA Equipment & Tools for the helicopter operations).
