UNIVERSAL EMPLOYEE ACKNOWLEDGMENT and CONSENT FORM
1. I have read and understand my job description
2. I have read and understand the Company Goals and the National Park Service Mission and I will endeavor to do what I can to add value to the lives of our customers, minimize our environmental footprint, and support my co-workers.
3. I have reviewed all sections of the Employee Handbook pertinent to my job function and know where I can access the Employee Handbook either on-line or in the office for future reference as needed.
4. I have read, understand, and agree to abide by all personnel policies related to the workplace such as, but not limited to, maintaining a drug-free workplace, preventing sexual harassment, and providing equal employment opportunity.
5. I consent to being tested for the presence/absence of illegal drugs in my body pursuant to the Company drug-free workplace policy. I understand that the Company will not share the results of any drug test with another employer without my prior written consent.
6. I have reviewed the Grand Canyon National Park Commercial Operating Requirements and I agree to comply with these regulations while performing my job.
7. If I handle hazardous chemicals as part of my job function (warehouse, guides), I have read and understand the Hazardous Communication Plan.
8. I consent to the use of any photos which contain my personal "on-the-job" image if needed for AzRA promotional purposes.
9. I participated in a pre-season orientation which included review and discussion of Company policies and procedures related to safety, environmental protection, workplace issues, and other matters.
10. I understand the company's goal around Respect and Inclusion, and will abide by the Code of Conduct.
