FUNCTION
To supervise Case Managers and provide Case Management Services to consumers
ESSENTIAL FUNCTIONS
1. Case Management and assessment of consumers assigned by Mission Services Director. This includes CCMEP.
2. Maintains complete records on all consumers receiving services as assigned
3. Performs intake for consumers
4. Perform Mental Health Assessments for CCMEP
5. Meets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database
6. Attends meetings and case staffings as required for each consumer
7. Initiates, maintains and documents service provided for consumers and updates in the consumer database
8. Coordinates daily consumer schedules
9. Prepares required reports on all consumers and trainees
10. Provides progress reports for the Mission Services Director on a weekly basis for each individual served
11. Works directly with consumers and trainees in all phases of the rehabilitation process
12. Assigns consumers to Case Managers
13. Manage Case Managers
14. Performs other duties as assigned by the Vice President of HR and Mission Services and the Mission Services Director
