Case Management Supervisor - Adult Services

IMMEDIATE SUPERVISOR: Mission Services Director

FUNCTION

To supervise Case Managers and provide Case Management Services to consumers

ESSENTIAL FUNCTIONS

1. Case Management and assessment of consumers assigned by Mission Services Director.

2. Maintains complete records on all consumers receiving services as assigned 3. Performs intake for consumers

4. Meets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database

5. Attends meetings and case staffings as required for each consumer

6. Initiates, maintains and documents service provided for consumers and updates in the consumer database

7. Coordinates daily consumer schedules

8. Prepares required reports on all consumers and trainees

9. Oversees the OMJ Lab Instructor

10. Oversees WIOA, OWF, ABAWD and Community Programs

11. Provide services to CCMEP TANF participants as needed

12. Provides progress reports for the Mission Services Director on a weekly basis for each individual served

13. Works directly with consumers and trainees in all phases of the rehabilitation process

14. Assigns consumers to Case Managers

15. Manages Case Managers, ABAWD Coordinator and OMJ Lab Instructor

16. Performs other duties as assigned by the Vice President of HR and Mission Services and the Mission Services Director

1. Maintains a good working relationship and provide excellent customer service to all consumers, staff, customers and supervisors

3. Complies with all established work rules, policies, procedures and safety rules

4. Represents the organization in a professional and courteous manner

5. Is able to educate stakeholders and community about the entire agency

 

 REQUIREMENTS

Positions Supervised: Case Managers. OMJ Lab Specialist, ABAWD Coordinator

College degree in related Human Service field or High School diploma or related work experience can replace degree

3 years experience in related field Experience: Certification: CPR/First Aid, LSW preferred

License Check - valid driver's license and proof of insurability or is able to provide transportation

Background Checks - The offer of employment will be based on the outcome of the background check

Drug and Alcohol Screen - Must pass a drug and alcohol screen prior to employment Subject to random testing
