IEP Teacher Summary -Elem K-6 Form
This form will provide valuable information in determining student IEP goals and accommodations. Please be as specific and detailed as possible.
Reading summary (Must include: letter they know, sounds they know, letters and sounds explicitly taught to date, list SKILLS they are proficient and not proficient in with the grade-level expectations).
Math Summary (List skills explicitly taught to date and indicate if student was proficient or not proficient) *
Writing summary (short paragraph outlining grade level expectation explicitly taught to date, the student's current writing ability, and how the student's writing sample compares to that of their peers).
List all accommodations/modifications this student is currently utilizing for testing.
List all accommodations/modifications this student is currently utilizing in the general education setting.
List any concerns (not already listed above), academic (do they attend intervention), gross motor, fine motor, speech, emotional concerns, or social concerns you have for the student.
