To be completed after user registration is approved for the current school year.
As a public school district, we offer the opportunity for community members, businesses and local organizations to utilize our facilities as a resource. The following pages will take you through the Facility Use Request process.

Applications must be received no less than two weeks prior to the event for application consideration. If you have a last minute request or change to an approved request, you are asked to contact the Facilities Department at 541-923-5437 ext 1166. Last minute requests are not guaranteed to be processed even when district staff are contacted. ANY NEEDS NOT SPECIFICALLY ADDRESSED IN THE APPLICATION FOR FACILITY USE ARE NOT GUARANTEED.

User Agreement/Acceptance of Responsibility
I agree to comply with all District Board Policies and District Building Rules and Regulations and will exercise the utmost care in the use of said facilities.
I understand the use of certain equipment may require supervision by District personnel and additional fees.
I understand that RSD programs have priority over other building use and this application may be subject to change.
I understand that I am responsible to follow up on the approval of this Facilities Request Form.
If this application is approved, I, an authorized representative, hereby accept responsibility for any fees and/or deposit payments, as well as any expenses for losses or damages. A written notification of cancellation of this event must be submitted to the RSD Events Coordinator 48 hours prior to the scheduled event in order to qualify for any partial or full refund of deposit.
