Thorndale Youth Clubhouse Feedback Survey
We are eager to learn more about the interests of the youth living in Thorndale as well as any barriers that might be preventing them from attending. If you have a young person in your home that is over 10 years old, we would appreciate it if you would complete this quick survey with them in order to provide us with the information we need to make adjustments and ensure that we are offering a relevant service to youth in Thorndale. Everyone who completes this survey will be entered into a draw to WIN 1 of 5 FREE memberships to the Clubhouse.
