    COVID-19 Vaccination Status Questionnaire
  • The Centers for Medicare & Medicaid Services (CMS), has announced an emergency regulation requiring vaccinations for nursing home workers, hospitals, dialysis facilities, ambulatory surgical settings, and home health agencies, among others, as a condition of participation in  Medicare and Medicaid programs.  

    As a Medicaid provider, Silver Home Care is obligated to comply with this regulation.

    Therefore, In compliance and effective December 5th, 2021 all employees will be required to have received the first dose of a two-dose COVID-19 vaccine or a one-dose COVID-19 vaccine to remain eligible for employment.  In addition, all employees must have received the necessary shots to be fully vaccinated – either two doses of Pfizer or Moderna or one dose of Johnson & Johnson – by January 4, 2022.

    If you have not received the vaccine(s), we urge you to do so now.    Effective Immediately we are offering the following incentives:

    1. $50 deposited to your account the next day for the first shot, if completed by November 15th 
    2. $50 deposited to your account the next day for the second shot, if completed by December 5th

    We will provide reasonable time off and paid leave for vaccination and any side effects experienced following vaccination.
