MAST TRUCKING
Current Policy and Acknowledgement of Receipt of Drug and Alcohol Policy and completion of online Employee Training for compliance in the Drug Free Safety Program.
All employees of Mast Trucking will receive a copy of the Drug Free Safety Policy and be required to complete the employee training annually. This form verifies that the employee below has received a copy of the policy and has completed the employee training on the date listed below.
Policy Overview:
Mast Trucking conducts drug testing which mirrors the current requirements and cut off levels of the Department of Transportation, 49 CFR and will be tested for the following drugs/alcohol:
CANNABINOID (MARIJUANA)
PHENCYCLIDINE (PCP)
COCAINE
OPIATES
AMPHETAMINE
ECSTASY (MDMA)
ALCOHOL
The cut off levels for the above listed drugs are listed in the full policy and procedure.Alcohol levels above .02 will be considered a positive test and subject to disciplinary procedures.
Mast Trucking will test employees in the following circumstances:
NEW HIRE
POST ACCIDENT
REASONABLE SUSPICION
RANDOM
FOLLOW UP
DISCIPLINARY ACTIONS

Employee Training
All employees are required to receive annual Substance Abuse Education compliant with the Drug Free Safety Program.
By Signing this form in the space provided below, you are acknowledging that you have received a copy of the Mast Trucking Drug Free Workplace Policy and Substance Abuse Education Training, speak and understand English and that you have had the opportunity to discuss the policy and have your questions answered. You further understand that it is your responsibility to read the policy in full and abide by all requirements of the written policy. Any violation of this policy will result in discipline up to and including termination of your employment.
