The UIWSOM Readmission Application is commonly used when a learner has been on an approved leave of absence for greater than 180 days within a 12-month period.

Please understand the following items must be completed prior to readmission or by the established deadline(s).

• All conditions of the approved Leave of Absence are resolved or completed.
• Pre-Check and Sentry MD requirements must be up-to-date and in compliance.
• Prior to registration, outstanding balances must be paid or the learner must be on an approved payment plan with the UIW Business Office.
• If the learner is repeating Year 1, all pre-matriculation tasks required of incoming students must be completed by July 1 including participation in all Welcome Week events. 

If a leave of absence exceeds 180 days within a 12-month period, the leave of absence may convert into and be deemed a withdrawal from the program. A student who is withdrawn under these circumstances must apply for readmission.

Factors relevant to readmission may include the student’s academic status, the nature and duration of the leave of absence, demonstrated academic progress or personal growth during the leave of absence, and commitment to completing the remaining program of study. Readmission is not guaranteed.

The decision to readmit shall be made by the Associate Dean of Medical and Interprofessional Education or Associate Dean of Clinical Affairs. To facilitate readmission, the Associate Deans may refer the student to the SPC or request that the SPC provide a recommendation on the student’s readmission application.

• I will ensure all Pre-Check and Sentry MD requirements are up-to-date and in compliance.
• I understand tuition may be discounted but is not guaranteed. I will communicate with the UIWSOM Director of Enrollment Services for details.
• If financial aid is needed, I will work with the UIWSOM Director of Enrollment Services and may be required to submit a Satisfactory Academic Progress (SAP) Appeal for future financial aid consideration.
• If applicable, I will resolve all outstanding financial obligations.
• If repeating Year 1, I will be required to complete the pre-matriculation tasks by the established deadline.
• I will inform the Office of Admissions and Student Affairs if I plan to attend the White Coat Ceremony.
• I understand I will not be issued new medical supplies, white coat or scrubs.
• I have reviewed and agree to these conditions of readmission.
