  • Thank you so much for choosing to have a color service done with me here at Salon M. To best serve you, I need a little information to get us started on the right color journey. 

    You've found this form through the website, so I want to assume you've read over the new guest package options, as well as my policies and rewards program. If you haven't had a chance, I encourage you to do so. 

    On this intake form, I'm only asking for your name and email address so I can quickly respond if needed. When you go to Vagaro(my online booking platform)to book, please fill out your address as well as your phone number there, as I will need those to be on file. If you've already made your profile, you can always go back and edit these fields.

  • My goal is for you to love your hair! If you have challenges with your color or cut, let me know within 7 days of your visit and I am happy to correct the issue at no additional charge to you if you're available within a 2 week window from the date of your original appointment. 


    Time is everyone's most valuable asset in my opinion. I do my best to accommodate everyone I can and I do that best by limiting my last minute cancellations. Since 4 hours is a big part of my working hours, any color appointment 4 hours or longer is subject to a $100 rebooking fee if the appointment is cancelled less than 48 hours before your appointment. This fee is to reserve that time slot and is not put toward your color service charges.
