Luxury Extension Consultation Form

Thank you so much for your interest in extensions. I am certified in Great Lengths Keratin Bond Extensions as well as Hand Tied Weft Extensions. If you haven't already, you can go to the extension information page on my website to familiarize yourself a little bit with these two methods.

Please complete the form below to create a base for our in-person consultation appointment. This form provides information that helps me provide the best customized estimate and experience for you based on your current hair situation as well as your hair goals. We can discuss additional questions you have at your in-person consultation appointment. Please fill out this form as accurately as possible.

After submitting this form, you shlild recieve a text within 72 hours to schedule. Please check your junk email as replies often get sent there.

Once application method, colors, length are decided and the deposit has been paid, we will set the date for install. This can be as quick as a few days if my schedule permits.

To book an install appointment: there is a non-refundable retainer of $300 that will be credited to your total pricing.

I understand and agree to the terms/policy of the consultation booking fee and the non-refundable retainer fee.

Cancellation Policy

There is a 48 hour cancellation policy. If 48 hours notice is not given, the retainer fee will be void and a new retainer fee will be required to rebook.

If cancellation occurs with 24 hours or less notice, 75% of the booked services pricing will need to be paid in order to book again. New retainer fee will be required to rebook.

No call/no show

Retainer fee will be void and the individual will not be able to book future appointments.
