SPRAY TANS BY MEGAN POLICY:

You mush be properly prepped for your appointment. Come with clean skin, without deodorant, lotion, makeup, perfume, or sweat.
Showering is not allowed during Mobile Tan Party appointments.
It is required for male clients (or clients with male genitalia) to wear underwear.
It is required for any client under the age of 18 to wear underwear and a parent or guardian must be present at all times.
Follow all instructions given during the appointment.
Be advised there is a small percentage of people whose skin may not react favorably to spray tanning. You must make known any allergies or substance sensitivities you may have before the spray tan process. For this reason, it is not advised being sprayed for the first time before an important event.
The results of spray tanning vary and the results are not guaranteed.
Any and all services given are non-refundable and non-trasferrable.
Spray Tans By Megan/Megan Griffiths reserves the right to refuse services for any reason before and during the appointment.
Caution - Pregnant or nursing women should consult their physician before getting a spray tan. It is not recommended to spray tan during your first trimester.
Warning - The products used during your appointment do not contain a sunscreen and do not protect against sunburns. Repeated exposure of unprotected skin to U.V. Light may increase the risk of skin aging, skin cancer, and other harmful effects to the skin even if you do not burn.
The spray tanning solution contains an active ingredient: DHA (Dihydroxyacetone). DHA has been FDA approved for external application only. When using DHA-containing products as an all-over spray or mist, it may be difficult to avoid exposure in a manner for which DHA is not approved, including the area of the eyes,lips, mucous membrane, or internally. Therefore, it is recommended that you take protective measures to eliminate eye contact, inhalation or during ingestionyour spray tan session, such as: a. Using protective eyewear b. Wearing nose filters c. Sealing lips with lip balm d. Using protective undergarments.

I hereby give my consent to allow Spray Tans By Megan/Megan Griffiths to apply all spray tanning products and related products.
To my knowledge, I have no medical condition or allergy which would preclude me from having this procedure done. I agree to hold harmless Spray Tans By Megan/Megan Griffiths of any medical complications that may arise and I have been advised to discontinue use if any reaction occurs.
I hereby release Spray Tans By Megan/Megan Griffiths and hold her harmless from any liability involved during or after the spray tanning process including during the spray tan developing stage. I accept sole responsibility if I have a reaction to any products. Spray Tans By Megan/Megan Griffiths will not be held responsible for damaged personal items or property during or after the spray tanning process.
I will provide a safe and respectful working environment for the spray tan artist.
I will provide a valid credit card to be held on file and charged in case of failed payment, cancellations/rescheduling fees, no show fees, and future payments.
I understand payment is due at the appointment and will make sure payment is given.
I give Spray Tans By Megan/Megan Griffiths permission to use any and all photos taken and given by me for marketing and educational purposes.
I have been honest and accurate about the information that I have provided on this waiver.

CANCELLATION AND DAMAGES POLICY

*CANCELLATION/CHANGES: Any cancellations or changes to the time of your appointment within 24 hours of the appointment time will result in a CANCELLATION FEE of 50% of service booked charged to the card on file.

*NO SHOW/LATE: There is a 5 minute grace period. If you NO SHOW or LATE more than 5 mintues of the appointment time, it will result in an immediate cancellation of your appointment and the card on file will be CHARGED FOR THE FULL APPOINTMENT BOOKED.

*DAMAGES: If you, someone at the appointment, or a pet damages the property of Spray Tans By Megan, the cost to replace the exact item will be added to your appointment and the card on file will be charged.

**Any Cancellation/Rescheduling Fees, No Show Fees, or Incidental Fees are non-refundable and non-tranferrable.
