Please go through this deposit contract prior to submitting your deposit payment. IF YOU HAVE A MOM'S GIFT CARD AND WANT TO USE IT FOR YOUR DEPOSIT PLEASE STOP NOW AND CALL THE STUDIO TO MAKE YOUR APPOINTMENT.

Note about Guest Artists: If you are scheduling with a guest artist, this means they are from out of town and will not be available for touch up sessions, if you need a touch up within the first 3 months of your tattoo, please contact the studio and we can assign a studio artist to assist with your touch up.

This deposit is with Moms Custom Tattoo and Body Piercing, not with a specific artist. In the event that your artist is no longer with the shop, we will transfer the deposit to apply to a new artist of your choice within the shop.

I understand that my deposit can transfer to another appointment time as long as I give the studio a notice of 48 hours (or more) before my scheduled appointment time.

I understand that if I cancel or reschedule my appointment with less than a 48 hour notice, I will forfeit my deposit and a new deposit will be needed in order to reschedule my appointment.

I understand that If I "no call/no show" I will lose my deposit and will have to leave a deposit in a greater amount than the previous deposit, which will be decided upon by your tattoo artist.

I understand that if I am more than 10 minutes late to my appointment, I may lose my deposit.

We cannot tattoo you if you are pregnant. If you are pregnant or become pregnant prior to your scheduled appointment, you need to contact the studio to reschedule. If you knowingly show up to your appointment pregnant and did not reschedule, you are subject to losing your deposit. By checking this box, you acknowledge this statement.

I understand that deposits do not purchase the artwork but are a down payment towards a custom tattoo.

I understand that if I change the subject matter of the tattoo then the deposit is compensation for the artist's time spent working on the original subject matter and a new deposit will be required for the new design.

If I have any healing issues or concerns within the first two months, I will contact my artist directly. I understand that my touch up may not be complimentary. If any touch up work to the tattoo is needed due to my own negligence, I agree that the work will be done at my own expense

We have limited space in our tattoo areas and for this reason ask that you only bring up to one guest and check in with your artist about the guest your bringing.

Use of pre-numbing products without approval from your artist may result in the loss of your deposit and a need to reschedule. Artists have the right to refuse service if pre-numbing is used in the area that will be tattooed.
