  • Liability and Medical Release

  • I,   *   *   ,understand that participation in dance and theatre activities, such as auditions, classes, rehearsals, and performances, could include actions or tasks which may be hazardous to the participant or volunteers. By signing below, I assume any risk of harm or injury which might occur to the participant or volunteer due to his/her/my participation in the activities of Triad Performing Arts of Arizona. I release Triad Performing Arts of Arizona from all liability, costs and damages which might arise from participation in these activities. If the participant is a minor, I agree that the minor has my consent to participate in these activities. I further provide my consent for Triad Performing Arts of Arizona to seek emergency treatment for the minor if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment.

  • I consent to allow Triad Performing Arts of Arizona to use photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), on Triad's website, and on social media (such as Triad's facebook or Instagram account). I further understand no compensation will be payable to me for use of these photos and videos.

  • Absences

  • List any dates here if you already know of potential rehearsal dates you will be unable to attend.

    Attendance is of utmost importance and excessive absences will be a cause for removal from the production. No more than 3 excused absences will be allowed.

    ALL absences must be communicated ahead of time with Ms. Bonnie or Mrs. Kat.

  • Volunteering

  • Every family is required to give at least 2 hours of volunteer time.  There are many options for this!  Select the areas below that you are interested in and our volunteer coordinator will contact you.  If you have other skills or talents to offer please write that in the comments. 

    If you would like to opt out of volunteering, you can instead pay a $50 volunteer fee (in addition to the $105 fee that everyone will pay for participation). Triad runs on the power of donations and volunteer hours, and we appreciate your support in one or both of these ways.

  • Required Participation Fee

  • The participation fee is an important part of our budget.  It helps with costumes, sets, venue rental, lighting and sound technicians, photography, etc. and the many rehearsal hours of our teachers. All of this makes it possible for us to put on a professional level production and create an amazing experience for our performers.
