For the use of life members or nominated delegates who are unable to attend the Gymsport Committee Annual General Meeting only.

NOTE: A proxy vote may not be given to a person who is not a member of the Association.

NOTE: Nominated Delegates filling out this form must be an approved club delegate.

In accordance with Part B Clause 5(K) of the Committee Policy a quorum at any General Meeting, which includes an Annual General Meeting and Special General Meeting, shall consist of 15% of the membership entitled to vote, present in person and/or present holding a nominated proxy.
