Dog Grooming Application Form

Hi New Friend!

Your pet’s safety is first here with me at Natural Pawz Pet Spa. I am by appointment only, so what that means is your dog will have one on one time with me and with no other dogs around. Your appointment time is blocked for a certain amount of time and I have other dogs scheduled throughout my day, usually back to back. So being on time is very important so I do not get behind on my schedule for the day and we don't have any colliding appointments. IMPORTANT: I only take dogs that are 55lbs and under AND I REQUIRE MY CLIENTS to be on a every 6-8 week grooming schedule if wanting a longer haircut. Thank you!
