Fill out the form below carefully to secure your dancer’s membership registration slot for open registration period, which begins on August 1st each year. We only conduct a one time registration each year. Each dancer is required to complete our academy 5 week training before being eligible for our elite competition program. Once registration is complete, a confirmation will be sent to your email including all information, tuition details and instructions a month prior to our registration period on your next following steps to joining the Exquisite Diamond Dance Academy or Exquisite Diamond Elite.

Welcome to the Exquisite Diamond Dance Academy and Exquisite Diamond Elite. We are glad you are interested in joining our dance family. Our program is skill based which is divided and consist of (2) programs. Our competition ELITE team program and our performance ACADEMY program. Our competition elite program consist of all variety of dance styles (specifically Majorette, Lyrical, Tumbling and Hip Hop) at a competitive level. All dancers are required to be at least 5 years old in age and must complete one full season in our academy performance program in order to qualify for the competition team. Our academy performance program consist of only Ballet, Liturgical, Hip Hop, Majorette and the Level (I) Tumbling. During the academy stage, each dancer is starting with the key basics of each style of dance and building technique foundation. All dancers are required to be at least 4 years old in age to register for any of the above program(s). The days listed are the following days each class or practice is held. Do you understand that you will receive all details, tuition information, requirements for attending class, class/rehearsal schedule and calendar of season to the email you have provided? Please be sure you have a valid email attached and allow at least 48 hours from the day of completing form for email to be sent.
Liability Waiver

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Exquisite Diamond Dance Academy. I/we agree to release and hold harmless Exquisite Diamond Dance Academy including its teachers, dancers, staff members and facilities used by Exquisite Diamond Dance Academy from any cause of action, claims, or demands now and in the future. I/we will not hold Exquisite Diamond Dance Academy liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Exquisite Diamond Dance Academy. I understand that Exquisite Diamond Dance Academy is licensed, accredited and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Executive Director, Artistic Director, Individual Instructor or staff member as soon as possible.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Exquisite Diamond Dance Academy has put in place preventative measures to reduce the spread of COVID-19; however, Exquisite Diamond Dance Academy cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Exquisite Diamond Dance Academy could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Exquisite Diamond Dance Academy and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to Exquisite Diamond Dance Academy affiliates, employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Exquisite Diamond Dance Academy or participation in programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Exquisite Diamond Dance Academy, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Exquisite Diamond Dance Academy, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Exquisite Diamond Dance Academy program. BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, PRIVATE LESSON, OR OTHER ACTIVITY THROUGH THIS WEBSITE, YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS. IF YOU ARE AN ATTENDEE OF OUR REGULAR CLASSES AND HAVE REGISTERED AND ENROLLED IN A CLASS THROUGH THE “PARENT PORTAL AND/OR STUDIO DIRECTOR” YOU AGREED THROUGH THERE AS WELL. PLEASE DO NOT HAVE YOUR CHILD/YOURSELF PARTICIPATE IN ANY CLASS OR EVENT UNLESS YOU ACCEPT AND AGREE TO ALL OF THE TERMS IN THIS AGREEMENT.

Membership Registration Fee
Membership registration fee secures your slot for our Academy program. Once complete, you will be officially fully registered as a dancer of the Exquisite Diamond Dance Academy and will then receive a email invite to join our parent communication group app. You may also submit your registration fee at www.exquisitediamondacademy.com under “New Membership Registration”. The deadline to submit this fee to guarantee your slot is October 8th. This fee is non-refundable. Please be sure that you will be attending before submitting fee.
