Prayer is an important part of Forge Equipping. As you prepare for your training with Forge, we’d encourage you to start a personal prayer team—a group of people close to you who will commit to praying regularly as you prepare for and participate in Forge Equipping. By giving us your prayer partners' information we can keep them in the loop during your discipleship equipping experience! Bellow please fill in the information for atleast 2 prayer partners. Commissioning Celebration Information

As your program concludes we will hold a commissioning celebration. So we would like to let you know we will be inviting your immediate family to attend. Having one or more of your family members in attendance to experience a taste of what God did in and through your life during your time here is a moment you will not be able to replicate. Plus, it will ease your transition back into your school/home/work environment as your family becomes more familiar with the transformation God has initiated! Please Give us a list of 1-5 people who you would like us to invite to the Commissioning Celebration.

MEDIA RELEASE: I hereby give consent to Forge and give them the right to make any recordings of me in the form of photographs, video or audio recordings. The recordings can be used presently and in the future with no purpose or time limitation. I further give consent for my identity to be released in association with the recordings made of me. FINANCIAL AGREEMENT: I am planning to attend Forge Equipping and agree to 1) be financially responsible for the tuition plus the cost of optional add-ons that I have opted in for. And 2) be financially responsible for any and all non-refundable expenses incurred on my behalf in the event that I should withdraw from Forge Equipping. STUDENT AGREEMENT: I have read through the entire "Policy Manual" and agree to abide by each and every policy outlined within. If I were to ever break any of the policies outlined in the Policy Manual, I understand that the Forge Equipping Leadership Team, under the accountability structure of Forge, reserves the right to take proper disciplinary actions according to my behavior.
