2022 Registration Checklist

All Items Listed Below are Required to Complete Registration. Pictures from your phone and/or scanned copies of these documents are both acceptable for registration.
Required Items *

Grade Verification (Image or document with player's name, grade and school year; eg. Report card or Powerschool screenshot) Copy of Birth Certificate Proof of Address (current utility bill with parent name and address) Current Health Insurance Card (2022 waiver may be submitted and is available on the SVYF website to print, sign and scan-or picture) 2022 Physical Form (must be turned in with registration.... NO EXCEPTIONS!) OBL Variance Form (must be turned in with registration if requesting.... NO EXCEPTIONS!) SVYF Code of Conduct (available on SVYF Website to print, sign, and scan-or picture) WFFL Agreement (available on SVYF Website to print, sign, and scan-or picture)

League Changes/Comp League

Please read the attached flyer and select if your child would like to participate in the new try-out based competitive portion of the league.
Grade Variation Requests *

My child does not wish to request any grade variations (Regular registration) My child would like to apply for an Older but Lighter (OBL) variation My child would like to request to play with 1 grade older if possible
Competition League Participation Request *

My child would like to try out for an "A" team if available My child would NOT like to try out for an "A" team if available(Regular registration)
