**Submitting this form does not mean your bathing reservation is confirmed. Please use our Appointment Request form and a member of our staff will reach out and confirm your pets bathing appointment.

To ensure that your pet's bathing experience is as pleasant as possible, please read and fill out the following:

For a safe and pleasant bathing experience, we would like to make you aware of the following bath policies:

Vaccines/Fecal Test - Rabies, distemper/parvo, and canine influenza must be current within the last year, and bordetella current within the last six months. A fecal test for internal parasites must also be current within the last year.
Parasites - Any pet entering the facility with fleas, ticks, or intestinal parasites will be treated by the veterinarian on duty to ensure our environment remains safe for all animals on the premises. The pet owner is responsible for the cost of this treatment.

SPECIAL INSTRUCTIONS FOR THE BATHER:

Some animals may require sedation to be groomed (due to aggression, fear or pain from excessive matting or arthritis I authorize the use of appropriate anesthetic agents and medications as are deemed necessary for the treatment of my pet. I further understand that there are certain risks involved with all types of anesthesia and procedures. I acknowledge that no guarantee has been made as of the results that may be obtained. We cannot guarantee that your pet will be groomed if we are unable to handle it safely. There is a $55 fee for sedation.

I assume all financial responsibility for all charges incurred relating to my pet's treatment and care. I understand that payment in full is due at the time my pet is discharged. If any fleas, ticks or other parasites are observed on my pet, I understand that my pet will be treated at my expense.

Our staff works very hard to have your pet bathed in a timely manner. You will be notified when your pet is ready to go home. Some pets may take longer than others, so if you need to pick up at a certain time, please let us know at drop off.
