Photo & Video Release Form

1. Consent for Use of Likeness  

By signing below, I hereby grant Whoop Em Wednesday permission to take photographs and video recordings of me during the event listed above. I understand that these images and recordings may be used for promotional, educational, and commercial purposes in print, online, or other media formats. I acknowledge that all photos and videos may be edited, distributed, and published by Whoop Em Wednesday at its discretion.

2. Commercial Use and Financial Disclaimer  

I understand that any revenue generated from content created, produced, and distributed by Whoop Em Wednesday will be solely attributed to "Whoop Em Wednesdays" and that I will not receive any financial compensation for the use of my likeness, voice, or performance in this content.

3. Liability Waiver  

I acknowledge that my participation in the event is voluntary and that Whoop Em Wednesday is not responsible for any injuries, accidents, or harm that may occur during the event or in connection with the use of photos and videos captured therein. I release Whoop Em Wednesday, its affiliates, and its representatives from any claims or liability arising from the use of the media and/or any personal harm or injuries incurred.

4. Duration  

This consent and release is perpetual and cannot be revoked.  

5. Acknowledgment of Understanding  

By signing this form, I confirm that I have read, understood, and voluntarily agree to the above terms. I am of legal age to consent or have a parent/guardian’s signature where required.
