I understand that if I book this service once a Date, Time and Number of Services is set it can NOT be changed within 30 days of wedding date.
I am aware that if someone drops out for hair services with less than 30 days notice I will still be responsible for payment.
I am aware that if my bridal party is running behind a late fee of $50 for every 30 minutes will apply. (example: 5 min-30min $50. 31min-60min $100)
I hereby consent to services booked with Studio H Salon. All information above was provided previously and fees will apply if changed with less than 30 days notice. I also give consent for photographs to be taken of Hair by Studio H Salon for promotional use. I certify that I completely understand and comply with the statements listed above.
