Grade Appeal

This form will be submitted to the Chief Academic Officer. In the case where the Chief Academic Officer was the course instructor, the appeal shall be submitted to the President.
When possible, disgreements between a student and a faculty member should be resolved informally. If the student believes s/he has not recieved a satisfactory resolution from the instructor or the instructor does not reply within five business days, the student may appeal to the Chief Academic Officer for resolution via this form.
