Consent Form
I,   First Name   Last Name  , Guardian of   First Name   Last Name  hereby grant and authorize Oseredok Ukrainian Cultural and Educational Centre and BORSCH Ukrainian Cultural Club the right to take, edit, copy, exhibit, publish distribute and make use of any and all picture and intellectual property of   First Name   Last Name, to be used in the Kids & Youth Writing Competition.
This authorization extends to all languages, media, formats now know or later discovered. The authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing. I waive the right to inspect or approve and finished product in which   First Name   Last Name appears. I understand and agree that these materials shall become the property of Oseredok Ukrainian Cultural and Educational Centre and BORSCH Ukrainian Cultural Club and will not be returned.
