Any information you share with us will be held in confidence between participant and staff/volunteers unless otherwise legally bound. If the life of a minor child is at risk, where suspicion and/or knowledge of physical or sexual abuse has taken place, or if we believe you are at the risk of suicide, or if we believe you intend to harm another person, we have a legal obligation to report this information to the necessary authorities.
Youth Unlimited (Toronto YFC), hereafter referred to as YU, will not disclose information concerning you and/or your child(ren) without first discussing it with you and obtaining your consent (except in cases where we are legally obligated, as stated above).
(a) Give permission to YU to collect, use and disclose reasonable personal information about myself and my child(ren) for the purposes of carrying out Young Parents Program activities and programs, including communications and registration requirements.
(b) Give permission for mine and my child(ren)’s photo and video images to be taken and used in the communication, promotion and financial support of YU events in perpetuity.
(c) Release YU staff and volunteers from all liability for what is discussed and/or information given.
