  • IMPORTANT: BEFORE YOU BEGIN....

    Remember to choose your classes carefully!

     Staff spend numerous hours, per student, to set-up schedules, follow-up on documents/fees, organizing teacher class lists, printing class materials and student name badges with individualized schedules, etc. 

    Class changes may cause the entire program to lose funding and in some cases, entire classes have to be canceled causing a teacher to lose a paid class. Changes in your decisions widely impact the teachers, staff and other students in the program. So please, take your time when choosing your classes, and if you have questions, please ask!

  • FINANCIAL OBLIGATIONS

  • It is imperative that you understand your financial obligations as it pertains to enrollment in Quo Vadis, and if utilizing state funding, with OVA.  To secure funding with OVA you/your student need to fulfill "count" requirements each semester. This varies by district. In essence, your student is participating in their classes on "count day". 

     A large portion of student funding is used up front by Quo Vadis for the purchase of classroom materials, supplies, curriculum, supplements, etc. If you choose to withdraw your student from the program before all funding has been provided to Quo Vadis, you will be held responsible for the amount that was not yet covered. 

      If a situation arises and you feel like you need to make a change, please reach out to Quo Vadis. We often have suggestions that can address your specific situation without adding the financial burden.

     

    In addition:

    1) If you request a class change AFTER you submit your registration, you will owe $25 per class change.

    2) If you request a class drop before classes begin, you will be charged $125 per class dropped. This fee is required due to commitments we have made to our teachers to run their class(es), rent the facility, etc. 

    3) If you drop classes BEFORE count month is complete, but after classes have begun, you will be financially responsible for ALL fees/charges associated with your child's classes. This could range from $240-$1500 PER class. These fees will be your responsibility as the school district will no longer fund selected classes without count month completion. 

    • I understand the financial expectations of myself and my family as outlined in the General Policies. In addition to the class change and dropped class fees, I, * , agree to either complete "Count" requirements each semester or pay all fees previously outlined for not fulfilling my obligation.


    • I, * , agree to return all materials to Quo Vadis at the termination of the school year or when I cease to attend, whichever comes first.

  • Please read through all the choices below and select the program that you are enrolling your student in. 

    Reminder:

    You need a separate registration per student.

    Only select 1 option per student, making sure all other option selections are on "None"

  • FUSION (CORE CLASSES - MONDAY/WEDNESDAY)

    Only make a selection in this section if your student is being enrolled in our Fusion program, covering Math, Lang. Arts, Science and History onsite each Monday and Wednesday during the school year.
  • ONLY ELECTIVE CLASSES

    Only make a selection in this section if your student is being enrolled ONLY in our elective classes. NOTE: This also applies to Full-Time students enrolled with OVA online but choosing to take electives at our location throughout the school week.
  • QV Fusion is a comprehensive program designed to meet ONSITE, at our Quo Vadis facility, two days a week (Monday/Wednesday). This applies to both Self-Pay families as well as our traditional, Full-Time district funded families.

    By enrolling in our Fusion program you are commiting for the entire school year of 2022/23.

    Parents are REQUIRED to work at home with their students to cover a full school days content on both Tuesday/Thursday each week.

    All students will be automatically enrolled in 4 core classes (Math, Science, History, Lang. Arts). Core classes remain the same for the school year.

    Students will also be able to choose 2 elective classes from our current offerings, taking place onsite Fridays and/or offsite throughout the week at Quo Vadis partners.

    Elective classes will be enrolled on a full year basis, but changes can be made 2nd semester if a student needs. (If students are taking an elective class offsite, they must choose from the Quo Vadis list; fees apply for offsite classes due to the cost of running them; this is the responsibility of the parent to pay - district funding does not cover these extra fees.)

    Math Placements tests are provided online for new students to help target any educational gaps. New students will be placed at grade level in all courses. If adjustment needs to be made after the start of classes we can address this concern with the onsite mentors to work on a solution to benefit the student.

    PART-TIME, K-12th grade students are required to enroll in 4 classes minimum with QVH*.

    Students enrolling in a full schedule (min. of 4 elective classes will get priority enrollment)

    If students are taking a class offsite, they are still required to take a 2 classes minimum onsite at QVH.

    *Exception: 9th-12th grade students who are enrolled FULL-TIME with a district are required to take 1 class onsite or offsite + $75/yearly reg. fee.  

    If students do not attend classes onsite, then the Service Hour Opt-Out Fee of $150 will need to be paid by the family.  Alternatively, parents can schedule service hours to avoid fees.

    If you are also participating in another district, the max you can use with OVA is 2.

    K-12th graders (where appropriate): Please make sure to choose both a 1st and 2nd choice. 

     

    DO NOT choose the same class for both choices. If a class becomes full, we will enroll your student in their 2nd choice.

     

    •  If your 6th-12th grade student will have a gap hour during the semester where they are onsite, unattended by a parent, then your student MUST be enrolled in Study Hall that hour and a $50 fee per gap hour/semester will be charged. 
    • Students in grades K-5 CANNOT have a Gap hour without a parent/guardian being onsite to monitor their child.  
    • Gap Hour/Study Hall can only be used between classes.

     

    Please know we will do our best to get your student in their first choice. 

     

    *Note: Our class enrollments are based off of your student's birthday.  If you feel your child requires alternate classes, please leave a message in the comments section at the bottom of this form explaining your situation.

    Part-Time Students: To be eligible to enroll in these classes, a minimum of 3 classes on-site at Quo Vadis must be on student schedule.

    • Please enter the shirt and pant size as well as weight and height for your child in the locations below (this applies to NEW students only).
    • Please also note if the sizes are girls, boys, mens or womens.

    If your child is a Returning Student, and requires a new uniform, please supply the below info. A $40 fee will be applied to your student balance.
