This request starts the three setp process for completing the OYWA requirement for a background check. Please fill out this form completely. When you hit the submit button it will be sent to the OYWA support desk.
Step one: You will receive an email with documentation of your submitted request form.
Step two: After receiving this email response you will be required to email or mail a signed letter from your school principal or superintentant stating that you have passed the background check for that district.
Step three: After emailing the required signed letter, please send a self-addressed stamped envelope for your badge to be sent to you.
Please remember the process ends on January 1st.
