Thank you for taking up a position within our Junior Blues program for the 2022/2023 VJBL season.
Each year, as a part of our preparation for the season we must complete the VJBL Coaches Screening process.
We have recently sent out the 2022/2023 VJBL Coaches Screening documents, these documents are also attached to this form below if you need to download them.
- Member Protection Declaration (Must be completed by all Coaches and Team Managers)
- Coaches Code of Conduct Form (Must be completed by Coaches only)

Please ensure you take a photo of the following:
- Valid Working with Children's Check or equivalent (Teacher's ID or Police ID)
- Completed Member Protection Declaration (Stat Dec)
- Completed Coaches Code of Conduct Form
