Vendor Application Form

Complete this form to apply as Vendor. Selected vendors will be advised by email, which will include a link to pay the registration fee. Please finalize your registration by submitting payment within 3 days of our notification email because space is limited and we want to provide as many applicants as possible a chance at participating. In case of late payment or missing vendor information, registration may be accepted at our sole discretion.
*Note: MLM businesses are not currently accepted as vendors.
Payments for vendor registrations are non-refundable, see refund policy on our website.

- Vendors offering food samples must obtain the required permit at the MAPAQ. If the event organizer is fined due a vendor's failure to obtain the required permit, the said vendor will be held liable to reimburse the fine.
- The venue requires that all vendors obtain liability insurance coverage of at least $2M for the event.
