Hall of Fame Nomination Form

The Bowls USA Hall of Fame annually honors individuals for their body of superior competitive achievements, and/or exceptional contributions to United States lawn bowling off the green.

Nominations to the Hall of Fame come from the Divisions.  Elections are conducted by the Bowls USA Council and announced during the Opening Ceremonies of the US Open.

This form consolidates relevant information for the Councilors to review in their consideration of a nominee.

It includes (1) lawn bowling participation and titles, (2) elected or appointed positions in lawn bowling organizations, and (3) other achievements and efforts in lawn bowling including those that may have been recognized by people outside the sport.

 

Titles

List the Nominee's titles with the year won, either individually or as a member of a team.  Include no more than 12 titles in each section.

Offices

List positions held by the Nominee, either by election or appointment.

Other Achievements

List the Nominee's other accomplishments on behalf of the sport of lawn bowls, i.e. committees, individual efforts, awards, and recognition by outside organizations.

I declare the division thoroughly reviewed the Nominee's titles, offices, and achievements and validates them to the best of their ability.

Note:  This form must be submitted to the Bowls USA National Secretary at least thirty (30) days prior to the next scheduled Annual General Meeting.
