Instructions and Notes

• You must declare a Primary Program of Study (first major) before you can add an additional program of study (second major), minor, concentration or Five College Certificate.
• You may change your Primary Program of Study (first major), but you must always have a Primary Program of Study.
• You may add a second major, minor or Five College certificate, or concentration at any time after you declare your first major. Smith College recognizes only two purely academic programs (major, minor or certificate) on the student’s record, in the following combinations:
  --One major
  --One major and one minor
  --One major and one Five College Certificate
  --Two majors (i.e., double major)
• A concentration is the only option to add a  third  program of study to your record. You must apply and be accepted into a concentration before you can declare it. For more information: https://www.smith.edu/about-smith/offices/registrar/majors-minors-concentrations
• Only one action at a time can be completed with this form. You may have to fill out multiple forms based on the changes you want to make.
• To change a program of study type--for example: PHY major to PHY minor
  --Drop the existing and add the new. In this example, you would drop your PHY major and then fill out a new form to add a PHY minor.
• To change your program of study--for example: BIO minor to ENG minor
  --Add the new and we will drop your existing. In this example, you would add your new ENG minor.
