ONT REPORT FORM

The vital Members the Ordination Nurture Team (ONT) provide a Candidate with ongoing support, guidance, and encouragement throughout their Ordination journey with the Chrisitan Church Disciples of Christ (DOC) of Mid-America.

This ONT Report Form [Appendix 7] documents a Meeting held with a Candidate and can be completed by any Member of the ONT. ONT Meeting Report Forms are stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM).
