The Christian Church (Disciples of Christ) of Mid-America
ONT REPORT FORM

The vital Members of the Ordination Nurture Team (ONT) provide a Candidate with ongoing support, guidance, and encouragement throughout their Ordination journey.

This Form is used to document an ONT Meeting, and will be stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM). This Form can be completed by any Member of the ONT.
