A Candidate for Ordination works with a Congregational Leader/Elder from their Sponsoring and Supporting Congregations. Each Meeting with a Congregational Leader/Elder is documented using this Report Form.

Report Forms are stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM).
