This Congregational Leader/Elder Report Form documents each Meeting between an Ordination Candidate and their Sponsoring or Supporting Congregational Leader/Elder. Both Congregational Leaders/Elders provide ongoing partnership and support by having monthly/bimonthly Meetings with the Candidate throughout their Ordination journey.

All Meeting Reports are stored in the Candidate's clergy file to be read and reviewed by Members of the Regional Commission on the Order of Ministry (RCOM).
