REPORT FORM

This Congregational Leader/Elder Report Form documents each Meeting between an Ordination Candidate and their Sponsoring or Supporting Congregational Leader/Elder. A Congregational Leader/Elder provides ongoing partnership through monthly/bimonthly Meetings with the Candidate during their Ordination journey.

This Form to be completed by the Candidate's Congregational Leader/Elder. Once submitted, the Form will be emailed to the Candidate for final review and approval. Once approved, this Form is remitted to the Mid-America Regional Office in Springfield. All Congregational Leader/Elder Meeting Report Forms will be stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM).
