All survey responses will be kept completely confidential. Once you hit submit at the end of this form your survey responses will be submited. We did add the option to include an email address with your submission - this will only be used to send an auto generated confirmation showing your submission was successful (with a copy of your input data) and then we will only contact you if there is a data error that needs correction. The emails will not be exported with the data for processing.
